- Read over all the text-based information located in the Information
Center.
- Carefully determine what type of procedure(s) are anticipated and print
them out.
- Select the appropriate providers by area (city, zip or county) and
specialty corresponding to the procedure(s) identified.
a. Review them one at a time for the aspects of their practice
that interests you.
b. After narrowing down the list, review the pricing and survey information.
c. If cost is a consideration, ask the provider or his business contact
to complete an agreement including the terms found in the attached (MyHealthScore.com Patient and Provider Fee Agreement Form).
- After your treatment is concluded, please fill out the on-line survey
forms for the provider and the insurance company.
